Breast Enlargement
Breast implants have been a focus of attention by the media and public for the past 10 years due to the controversy surrounding silicone gel implants. Recently these controversies were resolved and silicone gel breast implants once again became available for cosmetic breast augmentation. So now women have the choice of either saline or silicone gel implants for breast enlargement. The main advantage of silicone gel implants is that they feel more natural than saline implants. The second advantage is that saline implants can leak and lose their volume which would require the exchange of implants. Silicone gel implants do not have this problem. As a matter of fact one of the concerns of the FDA was leakage of silicone. For this reason silicone implant manufacturers re-formulated the silicone used in these implants to what is known as the "cohesive gel". This type of silicone does not detach even in the face of a rupture. Although various approaches are available, i.e. peri-areolar (through the nipple), inframammary (under the breast), and trans-axillary (through the under-arm), our preferred approach is the peri-areolar. It leaves an inconspicuous scar that is always hidden. By the same token we prefer to use smooth surface implants placed under the pectoralis major muscle. Personal experience has shown this to be effective in reducing the chance of capsular contracture (hardening of the capsule and breast) to very low levels. The commonly held notion that implants have to be changed every so often is only a myth. So long as implants do not cause problems it is unnecessary to exchange them. This procedure is perfectly safe no matter what type of implant is used (Silicone gel vs. saline) and especially in terms of mammography does not harbor any impediment to the visualization of the breast tissue. WHAT ABOUT THE BREAST WHICH IS SMALL AND SAGGING AT THE SAME TIME? Often times women have sagging of the breasts, as well as underdeveloped breasts, especially after pregnancy and nursing. The treatment usually consists of a breast lift along with implants.
